FILLING UP OF APPLICATION FORM

• Application form must be filled in and signed by the candidate in his/her own handwriting and duly countersigned by the parent or guardian.
  
  
• Application forms, which are incomplete or are received after the due date shall not be entertained.
  
  
• Application Form shall not be considered if the certificates/documents are not attested by the competent authority (i.e. the Principal of the Institute last attended by the candidate or a Gazetted Officer) with proper stamp and seal.

The application form must be accompanied by attested photocopies of the following certificates/documents:

1. Matriculation or equivalent examination certificate bearing testimony to the date of birth;
   
   
2. Detailed Marks Card of the qualifying examination, or evidence of having appeared in the qualifying examination;
   
   
3. Character certificate from the Principal of the College/School/University last attended;
   
   
4. Eligibility certificate in case of candidates coming from other Universities/Boards. Eligibility certificate is not required in case of candidates who have passed qualifying examination from The Universities of Punjab/Boards of Punjab/CBSE/ICSE;
   
   
5. Residence/Domicile certificate wherever applicable (see the Punjab Government Memo No. 1/3/95-3PPII/9619 dated June 6,1996 and letter no. 1/3/95-3PPII/81 dated January 1,1999 and further instructions issued by the Punjab Government, if any, on this subject);
   
6. Entrance test certificate with rank, wherever applicable;
   
   
7. In case of any discontinuity in studies, an affidavit (called the gap certificate) to the effect that the candidate was not involved in any offence or in any illegal activity and that no criminal case is pending against the candidate in any court, should be furnished by the candidate;
   
   
8. For reserved category candidates (see the Punjab Government Memo. No. 13/2/05-1TE2/409, dated 20.2.06 and further instructions issued by the Punjab Government, if any, on this subject) the application form must be accompanied by a certificate from the competent authority in the absence of which the application shall be rejected;
   
   
9. One recent passport size photograph duly attested by the Principal of the College/School last attended. In case of private candidates, this is to be attested by the Gazette Officer. Photograph must be affixed on the Form with glue (Please do not use staple pins)
